County EMS also handles dozens of long distance transports each week. All
long distance transports in and out of Leon County must be coordinated through
our office. To setup a
long distance or specialty care transfer, it is best to do so as far in advance
as possible. In order to best meet your transportation needs, we ask that
you fill out an EMS
Transport Request Form and fax it to (850)921-4100 along with a patient
demographic information sheet (FACE Sheet). Please obtain any required insurance
pre-authorization prior to setting up your transport. If you have any
questions about what else is needed, please feel free to call us at
(850)921-0900.
Additionally, if a patient is being transferred from a long
term care facility or hospital, you must complete and fax a physician certification form to
(850)921-4100, and give the original to the responding crew. You can also download
